Deadline for application EXTENDED: August 31, 2017
Please indicate the years in which you participated in activities or received honors.
Give your grade point averages (including all course work taken at each institution) for your undergraduate and graduate course work and include copies of transcripts from each enrollment.
The Applicant should provide an abstract of his/her completed thesis, dissertation or other research project using up to 500 words, including how it will benefit society with respect to hearing loss prevention/hearing conservation. (An addendum may be used). Students are highly encouraged to use the following subtitles in preparing abstracts since these criteria, along with overall clarity, will be used in judging abstracts:

• Rationale/Goal
Provide a strong and clear rationale for the research supported by the literature or community needs.
• Methods
The methodology and statistical analyses should be clearly described.
• Results/Conclusions
Clearly specify the results based on statistical analyses and add related conclusions.
• Importance of Work
Submissions which address an important issue or problem will be given special consideration. The potential impact of the research on clinical practice, education of students, the profession or, future research will be considered.
• Innovation (How innovative is this project?)
Projects which developed or employed novel concepts, approaches, methodologies, tools or technologies will be given special consideration.

You must submitt your official graduate studies transcript and two recommendation forms.

(a) Join and maintain a student membership in the NHCA at least until the student receives the
pursued degree
(b) Present the findings of his/her project at the upcoming or a one of the future annual convention of the NHCA. The award will be given at the conference where the findings are presented.
